After program enrollment, get Medical Clearance from your Physician or Physician
Weight Management Success Coach if you have any known, serious pre-existing medical conditions, or are on any
medications, which may require close monitoring or adjustment during the program as you lose weight and your health
improves.
Complete Initial Weight Loss Goal Profiler and Pre-test.
Then review each online Teaching Module and take corresponding post-segment Mini-Test.
Next, complete the post-course Profiler and take the end of course Post-test.
Finally, click on Teaching Modules and Forms, and Create and Print "Certificate of
Completion" for select course and now you are ready to begin our coaching and structured weight management program.
Begin the Weight Management Success Diet Program along with our or a graduated aerobic
exercise program and daily hydration
Follow up with your Physician Weight Management and Wellness Success Coach or PCP on a monthly basis,
particularly if your desire is to use FDA-approved appetite suppressants as prescribed by your program physician.
Follow up with MD after completing program.
Review at least one training course teaching module every other week
and follow up with your PWMS Coach with a minimum
recommendation of at least one module per month as you progress through the 6 to
12-month program and achieve your weight management and health improvement goals.
Review one weight management success education principle every 2
to 4 weeks, take corresponding self-assessment, then simply Download & Print Documentation
Form. Take form to your PWMS Physician/Coach or primary care doctor for
completion.
Commit to and stay focused on your Goal, remember your purpose for doing the program,
and adhere and submit to the strategy while you make your journey to Weight Management
Success. May make appropriate adjustments in physical activity and your dietary
practices along the way.
Have Physician refer you for Professional Nutritional Counseling (after you complete the
program as well as help you attain a 5-year weight history if you are considering
bariatric surgery). All DFL patient-clients should be referred for Nutritional
Counseling.
Have Faith and Confidence in Yourself and Your Ability to Achieve Your Goal!
Persist in actions and planning and continue on your journey until you Succeed!

Select, Refer to or Follow up with your Bariatric Surgery Center or Surgeon after 6-month Exit Medical Evaluation/Interview,
if considering surgery
Schedule and undergo Repeat Metabolism and Reassess Personal Calorie Budget
Continue with your Structured Meal and Exercise Plan until you reach your goal and beyond!
